Uniportal Robotic Assisted Thoracoscopic Surgery is being standardised at our center and we found it better tolerated especially so in high risk lung cancer cases. The video shows the technique where we use the camera in the center port than laterally. URATS has the advantage of easy conversion in an emergency, can shuffle ports at convenience and can use Robotic sure form staplers at any port without any difficulty and since its gas less, its tolerated well because of no excess intrathoracic pressure. But it requires an expert assistant like Dr. Vikas at patient side.
PORTS
We use three ports R 2,3,4 with camera in R3 monopolar scissors in R 4 and force bipolar in R 2. R1 is stowed or tucked away. Robot is docked from head side in X and from side with boom rotated. No targeting is done.The vessels and bronchus can be secured with 45mm Robotic Sureform Staplers with curved tip.
The assistants job is very very important and is always on the watch like our team mate Dr. Vikas
URATS is an innovative approach with potential to improve more as we understand the configuration and the movements of arms. We can modulate and develop it more in the coming times
